I give consent for the above named animal to be treated by Cherelle’s Veterinary Physiotherapy, using whichever modality is considered applicable including but not limited to laser, pulsed electromagnetic field, therapeutic ultrasound, mobilisations and manual techniques. Cherelle Veterinary Physiotherapy wil share all relevant information with the specified vet as given permission to by the owner.

Cherelle Veterinary Physiotherapy will not diagnose any conditions and will refer back to the vet if any new or undisclosed condition is suspected/found, only maintenance treatment will be carried out unless agreed otherwise.
